Please take the following steps to ensure your continuity of care.
1. Choose a new imaging provider. With multiple high-quality imaging services providers throughout our community, you have choices in your continued care. If you need assistance to determine your new imaging services provider, your primary care provider can provide a referral.
2. Obtain copies of your medical records. You may obtain copies of your medical records from HealthMark Group Customer Service: Phone: 800-659-4035, Fax: 469-676-8338. Requests may also be submitted electronically to HealthMark’s Request Manager at https://requestmanager.healthmark-group.com. Once logged in, select “Submit Request” from the menu options and enter all required fields to provide an authorization directly to HealthMark. Your medical record request will be processed, and a notification will be sent via mail or email once complete and available for download. 3. Complete final payments. Please ensure any final payments are received by November 15, 2023. To pay online, go to www.patientnotebook.com/scarsdale. To contact our billing office, call 281-972-7756. The billing office hours are: 8 a.m.-5 p.m. CT Monday-Friday.
